December Thank You Prize Draw

1. Eligibility

a. Entry is open to CheqUp Health customers ONLY, who complete the prize draw form and remain ‘Active’ at the time of the draw.
b. An ‘Active’ customer is defined as someone who is currently within a purchase or renewal cycle and has not paused or cancelled their subscription.
c. Only customers aged 18 or over and residing in the UK are eligible.
d. CheqUp Health staff members are not eligible to win
e. CheqUp Health reserves the right to disqualify any customers who are attempting to cheat by obtaining more than two entries OR any customers who have abused any other loyalty or discount scheme with CheqUp Health

2. How to Enter

a. Customers must enter via a link in an email from CheqUp and then complete the prize draw form, selecting a voucher preference and submitting it before the draw closes.
b. Customers must enter the draw using the same email address associated with their CheqUp Health account.
c. A second entry can be earned by answering the optional secondary question in the form.
d. A maximum of two entries per customer is permitted.
e. Any additional entries made above the two per customer maximum or entries from users who are not CheqUp Health customers, will not be counted in the draw.

3. Draw Dates

a. Entries open on Thursday 20 November 2025 and close at 23:59 on Wednesday 17 December 2025.
b. The draw will take place on Thursday 18 December 2025.

4. Winner Selection

a. Winners will be randomly selected from all eligible entries.
b. Customers must remain ‘Active’ on Thursday 18 December 2025 to be eligible to win.
c. Winners will be contacted within 7 days of the draw taking place.
d. We will not contact anyone who has not won a prize.
e. In the event a selected winner is not ‘Active’ on that date, a new draw will be conducted for that prize.
f. Only one prize may be won per customer.

5. Prizes

a. Winners will receive a choice of ONE of the following:

i. A £100 gift voucher for either M&S, John Lewis or Amazon
OR
ii. A £100 discount towards CheqUp Health medication

b. A total of 50 prizes are available.
c. Prizes cannot be exchanged or refunded.
d. If the prize is used to discount medication, the discount will only apply to medication and only the paid portion of the medication will be refunded if the customer later cancels their order.

6. Redemption

a. Details on how to redeem the prize will be sent by email to winners within 7 days of the draw taking place.
b. CheqUp Health is not responsible for the redemption or terms of use of external vouchers; customers must contact the relevant provider directly.
c. For CheqUp Health medication discounts, terms of use will be included in the winner’s email. Medication discount prizes cannot be used in conjunction with any other discount.

7. Data Protection

a. Your data will be processed in accordance with our Privacy Policy.

8. General

a. Only winning entrants will be contacted.
b. CheqUp Health reserves the right to disqualify any customer it deems in breach of these terms or acting dishonestly.
c. Prizes must be accepted as awarded and are non-transferable.
d. CheqUp Health may withdraw or amend the prize draw at any time in the event of circumstances beyond its control.
