CheqUp Clinical Waiver Criteria

The Early Cancellation Fee will be waived where the patient is ending their Commitment Plan because of a clinical issue, and one or more of the following applies:

  1. Side effects. The patient has experienced side effects, listed or unlisted, which the Clinician considers to be moderate or severe, persistent despite dose adjustment, or otherwise inappropriate to manage by continuing treatment.
  2. Contraindication identified after starting treatment. A clinical contraindication to ongoing treatment has been identified, whether through patient-reported information, new test results, or a change in the patient’s medical history.
  3. New medical condition. The patient has been diagnosed with, or is being investigated for, a new medical condition that makes ongoing treatment clinically inappropriate.
  4. Pregnancy. The patient is pregnant, breastfeeding, or actively trying to conceive.
  5. Interacting medication. The patient has been newly prescribed a medication that interacts with treatment, where alternative clinical management is not appropriate.
  6. Mental health concerns. There are mental health concerns related to or exacerbated by treatment that the Clinician considers material.
  7. Inability to tolerate. Despite dose adjustment, the patient is unable to tolerate the lowest clinically effective dose.

The fee will not generally be waived where the Clinician reasonably considers, having regard to these criteria, that the patient’s reason for ending treatment is not driven by a clinical concern. For example, the patient has changed their mind, is no longer pursuing the original treatment goals or has decided to use a different provider for non-clinical reasons.

Decisions are made by a registered Clinician and recorded in the patient’s clinical record. Patients who disagree with a waiver decision can ask for the decision to be reviewed by contacting [email protected]
