The Wegovy Pill is arriving in the UK this summer, and it comes with one requirement that is worth understanding before you start: a specific morning routine that directly affects how well the medication works. This post explains what that routine is, how the dosing schedule works and what to do if you miss a dose.
The morning routine
Unlike most tablets, oral semaglutide has an absorption requirement that needs to be followed consistently to get the full benefit of the medication.
Each morning, before anything else, take one tablet with up to 120ml of plain water (roughly half a small glass). Do not break, crush or chew it. Then wait at least 30 minutes before eating anything, drinking anything other than water, or taking any other oral medications.
That 30-minute window is clinically significant. The tablet uses an absorption enhancer to help semaglutide pass through the stomach lining into the bloodstream. Food, other drinks or other medications present in the stomach disrupt that process, and significantly less of the medication gets absorbed. This is the most common reason people feel the treatment is not working as expected. Often, it is not the dose. It is the routine.
Building this into your morning before breakfast, before coffee and before anything else is the most reliable way to make it stick.
The dosing schedule
The Wegovy Pill starts at a low dose and increases gradually over roughly 12 weeks. This gradual approach gives your body time to adjust and reduces the likelihood of side effects, particularly nausea.
The four steps are:
Step 1 (1.5mg): once daily for the first 30 days. Step 2 (4mg): once daily for the next 30 days. Step 3 (9mg): once daily for the next 30 days. Step 4 (25mg): the maximum dose, taken daily ongoing.
Each step up happens only when the previous dose is well tolerated. If side effects are making things difficult, your CheqUp Clinician may suggest staying at your current dose a little longer before moving up. Do not adjust your dose without speaking to your Clinician first.
If you miss a dose
If you miss a dose, take it as soon as you remember on the same day. If you do not remember until the next day, do not go back and take the missed dose. Simply continue with your normal dose that morning, as if the missed day did not happen. Do not take two doses on the same day to catch up.
If you miss seven or more consecutive doses, speak to your CheqUp Clinician before restarting. It is often better to step back down to a lower dose to avoid a return of side effects.
Side effects to be aware of
The most common side effects are gastrointestinal: nausea, vomiting, diarrhoea, constipation, indigestion, stomach discomfort and burping. These are most likely in the early weeks and after each dose increase, and tend to ease as your body adjusts.
Indigestion tends to be more noticeable with the oral tablet than with the injectable form of semaglutide. If you find it affecting you, there are a few things that can help: peppermint or ginger tea, over-the-counter antacids such as Gaviscon or Rennie, or proton pump inhibitors such as omeprazole or esomeprazole. If it remains difficult to manage, let your Clinician know. There are other practical adjustments they can suggest.
Staying well hydrated matters throughout, particularly if nausea is affecting how much you are drinking.
Other medications
Because oral semaglutide slows gastric emptying, it can affect how quickly your stomach absorbs other medications taken at the same time. If you take any medication where consistent absorption is important, make sure your CheqUp Clinician knows before you start. They will advise on timing.
If you take an oral contraceptive pill, raise this with your Clinician at your consultation. The same absorption effect can affect contraceptive reliability in the early weeks of treatment and after each dose increase, and your Clinician will advise on what to do.
